Can you run two tests to help determine what's going on?

1.) Remove the USB keyboard/mouse adapter and start the machine.  Do the
errors still occur?  You could either just let it go for a while, then
plug in the device, or preferably just login via ssh for the check.

2.) If the error messages stop, I agree it sounds like a hardware issue.
  You'd need to do a bit more research, but if the device itself was to
return that it supported high-speed mode (480MBps instead of the
full-speed 12MBps USB 1.1), I would expect those kinds of errors when
running on any USB 2.0 ports.  I've seen a lot of bad hardware that
didn't properly implement the USB protocol; this could be another
example, especially if the adapter is not a name-brand.  Could you try
disabling USB 2.0 mode and repeating the same test?
