I don't use Kaffeine but maybe I can help.

QAM 256 is the signal you get straight out of the broadband RF cable 
that most cable companies provide. (If your cable company uses a 
high-degree of compression in may be a QAM64 signal.)

I'm not sure how you are hooking your system up. There are at least 3 ways.
1. to the RF cable, which is most likely a QAM 256 signal (some of the 
channels are encrypted, some are not)
2. to your cable box, which provides an /analog/ output signal sent out 
through one or more of the following ports/jacks:
     Composite video
     Component video
     S-Video
     RF broadband cable
3. to an over-the-air antenna, which is a digital, ATSC signal.

Assuming that you're hooking directly to your cable company's cable, 
then I think the us-Cable-Standard-center-frequencies-QAM 256 should 
work. The trick is, if one frequency file doesn't work try another until 
you find one that matches what your cable company is sending. Sometimes 
the quickest way is to just try things until they work.

If you are interested in OTA (Over-The-Air) signals, then you can see 
which stations are close to you by going to TitanTV.com and entering 
your zip code and look at the TV schedule. Then you can go to 
http://www.antennaweb.org/aw/welcome.aspx, click on "Choose an antenna" 
and fill out the form, and it will show you exactly where every OTA 
station is in relation to where you live.
